What is IVF?
In vitro fertilization (IVF) is a common treatment for couples coping with fertility issues. This well-established procedure involves fertilizing eggs outside the woman’s body and then placing embryos into the uterus. Many couples consider IVF after trying to conceive for a year without success. It's also effectively used to prevent passing on serious genetic problems to children.
IVF works by stimulating the ovaries with hormones to create an optimal number of follicles. From these follicles, mature oocytes, or eggs, are retrieved. In the embryology lab, the collected eggs are fertilized with sperm, either naturally or using specialized laboratory methods. The fertilized eggs are then transferred back into the woman's uterus during the embryo transfer procedure.

The IVF Process and How IVF Works
Initial Consultation and Necessary Examinations
Based on a consultation with our experts and the results of your initial examinations, our experts will recommend specific methods suitable for you and prepare your comprehensive treatment plan.
Hormonal Stimulation
The purpose of hormonal stimulation is to obtain multiple oocytes, ideally between 8-10. We also seek to prevent spontaneous ovulation and other risks. It involves daily injections under the skin over 2-3 weeks, while continuously monitoring for risks such as ovarian hyperstimulation syndrome.
Egg Retrieval (Ovum Pickup)
During the woman's examination, we assess the function of her ovaries. Egg retrieval is conducted under general anesthesia and ultrasound guidance. Subsequently, the doctor collects follicular fluid containing eggs from the ovaries. In the laboratory, we separate these eggs from cumulus cells (which help the egg to develop) and assess their quality and maturity.
If using the woman's eggs is medically infeasible, we suggest IVF treatment with donated eggs. Couples can select a suitable donor with our help.
Sperm Collection
During the man's examination, we conduct a spermiogram test to assess fertility. This test helps us choose the right method for egg fertilization. If the results are good, we can use the sperm to fertilize eggs, or freeze it for later use. If sperm is not found in the ejaculate, we suggest seeing a urologist.
Based on the urologist’s results, we determine the next steps, which may include surgical sperm collection techniques like TESE or MESA. If the spermiogram shows poor results or viable sperm cannot be obtained through surgery, we recommend IVF treatment using donated sperm. Couples can choose a suitable donor from a sperm bank database.
In Vitro Fertilization and Embryo Culture
The eggs retrieved and sperm collected are combined in a laboratory dish for fertilization. Fertilization can happen through traditional insemination or intracytoplasmic sperm injection (ICSI), where a single sperm is directly injected into an egg. Embryos are then cultured for 2- 5 days, during which viable embryos are carefully selected for transfer.
Additionally, we offer advanced techniques such as PICSI and MFSS, among others, to further enhance the fertilization process and increase the chances of successful embryo development.
Embryo Transfer
Embryos are painlessly inserted into the uterus under ultrasound guidance. During the transfer, we use one embryo (or a maximum of two embryos).
Extra embryos that are not transferred may be frozen (cryopreserved) for future use.
Luteal Phase Support:
Hormonal medications, such as progesterone, are given to support the uterine lining and to increase the chances of implantation.
Post-implantation Checks
A blood pregnancy test is scheduled 10-14 days after embryo transfer. If IVF with own oocytes is not viable, IVF with donated oocytes can be considered.
Who should use IVF?
IVF is recommended in various cases, such as:
- Couples experiencing difficulty conceiving without a known cause
- Women diagnosed with fallopian tube obstruction or endometriosis
- Women experiencing issues with ovulation and who have not conceived despite using antiestrogens
- Couples with male sterility factors
- Cases where a specialist geneticist recommends genetic testing of embryos due to a higher probability of transmitting serious hereditary diseases
Additionally, IVF may be part of special programs involving oocyte or embryo donation, surrogate motherhood, and preimplantation genetic testing (PGT).

What factors influence the success of an IVF cycle?
Success depends on various factors, including the woman's age, ovarian reserve, embryo quality, and the cause of infertility. Clinical protocols, experience, and technology also play crucial roles.
If I live in another locality, how long will I need to stay in Bucharest?
Treatments can be coordinated remotely, and no overnight stay in Bucharest is required. International patients should plan for at least a 2-week stay, with a pre-organized treatment plan.
Can freezing affect embryos?
Some embryos may not survive the freezing and thawing process. If most cells recover after thawing, the embryo can continue to develop. The survival rate depends on the embryo's stage and quality, the cryopreservation method, and the specialist's experience. At the Gynera Fertility Clinic, over 95% of blastocysts (the ball of cells formed 5-6 days after sperm fertilizes the egg) resume development after vitrification, with over 50% resulting in successful implantation.
Do children born with IVF have an increased risk of birth defects?
The general population has a congenital anomaly risk of about 2-3%. Factors such as age, lifestyle, and pregnancy conditions can influence this risk. IVF does not significantly increase these risks, but it does not eliminate them. In cases of severe genetic male pathology, IVF/ICSI might slightly increase the risk of transmitting genetic abnormalities.
When is in vitro fertilization required?
IVF is required when a woman has obstructed or absent fallopian tubes or in cases of severe male infertility. It is also a solution for endometriosis or unexplained infertility if other treatments have been unsuccessful.
When is genetic testing of embryos done in the laboratory before implantation?
The genetic analysis of embryos is possible but not routine due to its invasiveness. It is recommended in genetic risk situations. It can also be a good selection option before ET (embryo transfer) in special situations, such as advanced age, high-risk cases, genetic disease in the family, or after repeated failures.
Is the chance of getting pregnant higher in the case of blastocyst transfer?
Embryos that reach the blastocyst stage (5-6 days old) have a higher chance of implantation. Most embryos do not reach this stage and stop developing after 3-4 days. Those that do reach the blastocyst stage have a higher potential for implantation.
How are embryos frozen?
Embryos are dehydrated to prevent ice formation during freezing. As water is removed, it is replaced with a cryoprotective medium. The embryo is then placed in a small container and rapidly cooled through vitrification. Frozen embryos are stored at -196°C in liquid nitrogen.
How are embryos thawed?
Thawing is the reverse of freezing. Embryos are heated from -196°C, and the protective medium is gradually replaced with water until complete rehydration. They are then evaluated and incubated under special conditions.
Is it dangerous if many follicles develop as a result of ovarian stimulation for IVF?
Developing over 15-20 follicles can increase success chances but also the risk of ovarian hyperstimulation syndrome (OHSS). Appropriate measures during treatment can mitigate these risks.
Is an IVF pregnancy different from a natural pregnancy?
IVF pregnancies have a higher likelihood of multiple births if more than one embryo is transferred. A single embryo transfer is recommended to reduce complications associated with twin pregnancies. IVF pregnancies may also show a slight reduction in birth weight, particularly with fresh (not frozen) embryo transfers. The risks of miscarriage, ectopic pregnancy, or fetal abnormalities are similar to those in natural pregnancies. IVF does not significantly alter these risks.
Can I travel during assisted reproduction procedures?
Travel by car, train, or plane is allowed at all stages, with a doctor's guidance on any additional necessary measures.

How long does the IVF process take?
The IVF cycle itself only lasts about 4 weeks, but the entire process from your initial consultation (including your pre-conception health routine and fertility testing) to your first beta-hCG test (to see if you are pregnant) can take up to 2- 3 months.
What is the age limit for IVF?
The maximum age is 45 years old for IVF with the woman’s own eggs, and 50 years old for IVF with donated eggs.
Are there any side effects/risks associated with IVF?
While fertility treatments are typically considered safe, it is important to acknowledge that all medical procedures carry a certain degree of risk when it comes to potential side effects. For IVF, these side effects may manifest as reactions to the hormonal fertility medications administered during the preparation phase for egg retrieval. Additionally, it is worth noting that there is a small possibility of encountering rare complications during both the egg retrieval and embryo transfer procedures.
